Your insurance may provide coverage for majority of our visits together. ​Please note because I am in network with your insurance, it does not guarantee automatic coverage. You are responsible for the cost of sessions if your insurance does not cover.

• Are there restrictions on which diagnoses are covered?

• Is diagnosis code Z71.3 (dietary counseling and surveillance) covered, or is a medical diagnosis required from my provider*?
  *If a medical diagnosis is required, please have your doctor fax recent visit notes and your diagnosis list to our fax number listed. 

• Do I need a referral or prior authorization from a primary care provider?

• How many visits are covered under my benefits?

• Do I need to meet a deductible before insurance pays for nutrition services? If so, will the deductible be waived for preventive nutrition therapy?

• Will I have a copay or coinsurance for nutrition services?
